The AZDOHS Public Affairs Office is committed to responding to all requests for identifiable public records in a timely manner. To obtain records in the custody of AZDOHS, please use the following procedures:

Walk-In Request
You may make a walk-in request at the State of Arizona Department of Homeland Security lobby where you will be asked to complete a Request for Public Records form.
About Records Requests
The State of Arizona Department of Homeland Security has specific policies and procedures regarding the release of identifiable public records which are in compliance with the State and Federal laws that regulate public disclosure with privacy rights. Costs for Public Records
The State of Arizona Department of Homeland Security has adopted the following charges for public records in accordance with Arizona State Revised Statutes. See the Public Records Request Form for more detail.
- Inspection: There is no charge for inspecting public records.
- Electronic Records: When available, electronic records do not have a charge associated with them unless they are requested to be put on disk or other media.
- Black and white copies (8 ½ x 11) are 25 cents per page.
- Mailing includes actual cost of postage and shipping container.
- Other media such as disks, audiotape, photographs or videotapes, will incur the actual cost of the media.
- Large Requests may incur a deposit of up to ten percent of the estimated cost of an entire request before copying records.
