**MS Excel 2007 Users: Follow these instructions when completing the Standard Data Collection Form**
- Save the Standard Data Collection form before opening it.
- When you open the spreadsheet, a security warning will display directly under the ribbon at the top of the screen.
- Click on the “options” button to the right of the security warning and then select the “enable this content” button and then click “ok”. This will allow for you to select options from the drop down menu boxes.
- You will have to follow steps 2 & 3 every time you open this spreadsheet.
